Technically speaking, there is currently not a way to upload your own file to an application form to be downloaded by your users.
However, there is a way to have your file available to be downloaded by your users. We can turn a link to a file you have saved on your website into a clickable download link.
Simply follow these steps:
STEP 1: Contact your webmaster or IT department. Provide them with the file(s) you want added to your form and ask that they save the file(s) to the website files. Ask that they return a URL to the file(s).
STEP 2: Choose one of the two methods to add this file to your form.
Option 1: Send the URL(s) to our Support team via a Support ticket. Be sure to let us know exactly where in your form you would like each URL you send to us. Also, let us know what text you would like for the link to each file. Typically, that would be the name or description of the file or, simply, "click here to download".
Option 2: Add the URL(s) yourself. To do this, you need only to use some basic HTML tags. Insert the following into a question text (typically you'll select an Instruction question type) where you want the link to appear.

Once you save your newly edited question, you will see a clickable link where the HTML tags were. Click this link to ensure that it goes to the correct location and downloads the file via your browser (each browser does this differently).
